My organization needs to conduct an application portfolio review. I know that application rationalization/modernization efforts are seen as a complex and time-consuming process. For those of you in this community who have gone through this before, how did it go, and what do you wish you knew that you didn't know before?

Indeed, that can end up in a cumbersome exercise if done on the most granular level (system, functionality).For each application cluster it can be sufficient to map its main capabilities. Eg transport planning as a capability for a tms cluster.
That is a business exercise with enterprise architecture. The capabilities shall be mapped to the overall capability roadmap of the company. (EG business should know if this capability will be sufficient to cover via erp or will it go for a tms system cluster)
If this prerequisite analysis does not exist, it will be from an it perspective very difficult to evaluate the data center requirements. Please let me know if you require further information. Happy to help.
