Be honest: are off-hours meetings (specifically meetings before 8 AM or after 5 PM) ever acceptable?

Yes. But attendees are often consulted, and group agrees.
1) project work that involves folks in different time zones may be approved
2) Physician meetings
3) 7/7:30am is more common than after 5pm

If acceptable or not, they are scheduled. I make decisions about attendance based on certain factors. Emergencies happen. I just deal with it.

They are, all depending of the context (emergency etc). However for me it has to be the exception, and not the standard.
