It's hard to get a decent system documentation.   We're quite good to write documentation at a project scope.   But having a real, useful and uptodate system document is challenging.

Most of the time, I'm surprised by the poor quality of it.   And worst, depending on the team, the documentation is stored in many places (Confluence, Sharepoint, etc.).

There is always scope for improvement when it comes to documentation; the same is true for us.

Documentation is always a WIP. Everyone should make honest effort to keep updating it so that it stays current.

There is always room for improvement, and whenever new functionalities are rolled out, it is imperative to update the documentation to stay current. Maintaining up-to-date documentation is a continuous process with no end state, and it is the responsibility of the entire team, not just one person.
