What are some tactics to provide feedback to our low performing team members? I want to be empathetic and constructive.

Rather than stating "you're good at this but not at that", frame it as an observation to start a discussion: "I can tell you enjoy a lot doing [domain they're good at] but I'm not sure about [domain they're weaker on]. What do you think?"

The most important thing is to really help the team member understand the reason behind the feedback as well as the impact that their performance is having on other team members.
