How can incidents like the recent train derailment in East Palestine, Ohio be prevented in the future?

Maybe look towards Europe and Asia where they have modern and efficient rail transportation as an example. Our rail systems are old and failing.
They could tow a lot of equipment that scans the track, measure bridge deflection, steel wear etc.
If it was a car failure, perhaps more onboarding mesh network for monitoring each car could be implemented to warn when odd noises start or temperatures start to climb on equipment
