Making sure you have a safe place for those conversations to happen; making sure you have a good number of visible mental health first-aiders; role-modelling the discussions by having senior leaders discuss their own challenges and how the company has helped them; etc.

There are lots of ways, but I was a bit concerned by the first part of your question which was how to 'identify' them - just that approach alone creates an unsafe space - I wonder if you need to start with an examination of why it's important to the business, and put in place a learning program first?

I think it is simple: be vulnerable, create a safe workspace to share struggles, demonstrate empathy and compassion, and strengthen one-to-one relationships to open these types of conversations.
