How does your business identify the problems that are holding it back?

I do brainstorming sessions with my teams focused on identifying challenges that are holding them back and that lead to defining future projects on how to overcome them. We sometimes draw/picture the ideal posture we want to be and then go back to where we are at, then define what is missing and stopping us. We use the feedback of other teams, data on current and past project successes and failures, we look at past reported issues, outages etc.Managing Partner, Partnerships & Strategy in Software3 years ago

Is anyone in the business able to submit issues to the leadership meeting, or is it only those who have a seat at the table?

Anyone can bring up issues. That said we expect our divisional and team leaders to be accountable in spotting, solving, escalating. The issues we solve for in our weeklies are the big rocks that impact the whole company.
